 Abstract:
Blood utilization has become a major concern for a variety of reasons.  Blood costs have more than doubled in the last few years and blood shortages are becoming more frequent.  New evidence about transfusion therapy shows that the risks of transfusion have generally been underestimated and the benefits have been generally overestimated, leading to an imbalance in risk-benefit decisions.  Since most medical schools and residencies do not adequately cover blood component therapy, it is imperative that physicians who order blood products be familiar with the current evidence regarding the physiology of stored blood and the non-infectious serious hazards of transfusions, as well as controlled studies of transfusion effectiveness.

Objectives:
At the conclusion of this lecture, the participant will be able to:
1. Detail the evolution of evidence based transfusion studies over the past two decades
2. Describe the “storage lesion” for blood products and relate that to clinical outcomes
3. List the current major risks of blood transfusions
4. Construct a risk-benefit strategy to balance anemia risks vs transfusion risks

Goal:
The goal of this presentation is to review the current evidence-based management of transfusion therapy, with an emphasis on red blood cells.

Teaching Strategy:
The teaching strategy will be didactic in the form of a PowerPoint presentation, followed by a question and answer period.
